Since (I believe) v 14.x, I habr been getting this message. The reason is unclear because my MacOS Privave and Security settigs for Lightroom Classic is "Full Disk Access"

If I click {OK} it goes away but may return several days later. Is there a solution? What might be the cause?

Since the various recent upgrades to both the Mac OS and LrC I’ve been getting these too. No idea why. I made sure LrC and Creative Cloud has access to all the folders and removeable drives and has full system access.

is it a MacOS bug or a LrC bug?

Have not seen on Windows so probably some mis-match between Mac security and LrC.

But, can't imagine that "Full Disk Access" wouldn't cover it unless Apple suddenly re-classified some file or folder as some sort of protected system file/folder.

Could LrC be relying on some other program to do some operation that runs outside of the LrC process thus also being outside the "All disk Access" umbrella granted to LrC?

The app is on the primary volume and the catalog and image files are on an EHD. Since MacOS 15, the refresh rate has be very sluggish. It is possible that LrC requests access to a file and MacOS being so slow doesn't respond in time So LrC thinks it does not have access.

Full Disk Access does not mean that Lightroom Classic has permissions to write to each and every folder on disk. There is still folders, such as system folders and folders of other users for example, that are off limits even with this permissions setting. This dialog usually means that Lightroom Classic does not have access to some user/library folder or some root/library folder, possibly because folder permissions got scrambled up. Have you tried the script that Adobe supplies? https://helpx.adobe.com/lightroom-classic/help/allow-permissions.html

That script is out of date. This is what is available in MacOS 15. And Yes I did check access on File and Folders the first time I encountered the message.

And from the Website:

The script may be out of date, but that doesn't change the rest of what I said. Having 'Full Disk Access' does not mean Lightroom Classic can write to just any folder on your hard disk. Some folder are (and should be) off limits, other folders may be off limits while they should not be, because of wrong security settings.

Except it is now not possible to add additional folders once the "Full Disk Access" has been selected.

Ah, I thought you meant that you could not add folders in Lightroom Classic. Full Disk Access is higher up in the chain than Files and Folders, so if an app has Full Disk Access, then Files and Folders becomes irrelevant. At least that's how it was in MacOS Sonoma (that I still use).

I've noticed that many people miss the "apply to enclosed items" part of the instructions. Most frequently, it seems to be caused by Develop presets that were installed by an installer like MastinLabs and similar. That said, I saw it yesterday, and then it disappeared and hasn't been seen again since, so something a little buggy does seem to be going on.
