too may permutations of keywords and naming

Anyone out there who has a well developed approach to using keywords, labels, captions, titles, etc?
I've been using place names as keywords (we were just in Prague, Vienna, Florence and rome) with the names of churches as keywords inside of the city keyword. BUT, maybe I should use the CITY field instead?
Yeah, yeah, I know it doesn't really matter, but I'm sure someone has put energy into this and I'd like to benefit from that experience.

I usually put names of churches into captions.

My approach is that keywords are words I'm going to use a lot. Captions are for details that are specific to that photo, or a few photos. But there's no concrete rule as to where to draw the line. Whatever works for you.

If you do an internet search, you can find several well thought out Keyword heirarchies some for free and some for fee. One of those is by a frequent poster here

You can begin with these or modify some of them to suit your style.
One of my high level keywords is "locale". I don't define the US as a country, but I do list each state. And if you drill down far enough, you will find my "Backyard" at the lowest level in "Locale" for those photos taken there. I don't use the IPTC fields for location because they are not well integrated with the Keyword schema. For me it is easier to List the Keywords Europe>Netherlands>North Brabant>'s-Hertogenbosch>Railway Station as a heirarchial keyword than to list Just the City, State/Province, Counrty in the IPTC fields and Not have a place for 'Europe" and Sometimes "Location" is not a clean fit for the lowest level. If I had the capability, I would used GPS coordinates for Location and only include important keywords like "'s-Hertogenbosch" and not have to look up what province it might be in.
