Suspected Duplicates - What exactly is the criteria?

In import dialog, there is "Ignore suspected duplicates" option. I need a bit of clarity on how exactly is a file determined as a duplicate.

I have "zero" out many SLR's (as in IMG9999.JPG went back to IMG''''1.JPG) so technically I will have quite a few duplicate filenames. Does lightroom distinguish these files are new files or duplicates? Is it just keying in on filenames or does it match bits or dates or something else in the file's metadata? Can I trust lightroom enough not to discard unique files on my drive.
