Why do I suddenly need to sign in to Adobe with the Application Manager for a Lightroom trial? I have a perpetual license for LR 6.

It’s possible that your machine got deactivated somehow, or that you intentionally signed out at some point. You need to be signed in to use Lightroom 6 so just go ahead and see what it asks next. At the end of a trial, it’d give you chance to enter the serial number again if it somehow got lost.
