when deciding to open an image in photoshop from LR, how does one get photoshop to open the RAW file in ACR?

Since LR is essentially ACR with extras you are already in ACR. However, if you want to open a file in ACR then you must choose Open as Smart Object in Photoshop from the edit in options. Once it is opened then double-click on the thumbnail in the layers palette. That will then open the image in ACR. That's the most direct route.

OK thanks - what the difference between edit in/open as in> photoshop and a prest to export>photoshop?

. Using Edit In, no intermediate file is created.
No file is created until it is saved. I think that is only for CS4 and that with CS3 a file is always generated unless "original" is selected.

Nope, works for CS3 too, as long as you've got ACR 4.6, as it's fully compatible. It's CS2 that always generates a file.
