Noise Reduction Programs used with Lightroom

If I understand you correctly, no. Any image processing programs designed to work as plug-ins with Lr, actually operate on files which are rendered/rasterized and exported from Lr and then returned to Lr as such.

The Lr team has yet to develop or implement SDK (software development kit) technology which allows external programs access to the actual raw rendering pipeline.

Although not feature complete in the beta, there is new noise reduction technology to be included in Lr3. Other external programs, such as Noise Ninja, are available to work as 'Edit In' applications.

Some background info is available here: http://blogs.adobe.com/lightroomjournal/2''8/'8/plugin_or_external_editor.html

I have Noise Ninja. All it take is about two extra key strokes to shoot off the image to Noise Ninja. Once, you are done. Noise Ninja automatically sends back the image to LR. In LR, you will see before and after images side by side. I am quite impressed with the software. I felt the hard part was the setup. I followed someone's step by step instructions on YouTube.
