LRB PHP FORM

Hi, this is probably only a question Sean McCormack can answer. I recently bought LRB Portfolio, everything has gone well bar PHP which I know Sean mentions it doesn't always work in his video, but I have direct access to my host (just Host) and I can even edit the php script so wondering if there is a way to fix the php form?
