Import Imports already imported files

When importing files I click " Don't Import Suspected Duplicates" yet the preview shows a checkmark in the selection box that indicates a photo already imported will be imported. Is this common or something wrong with the way I have things set up?
 
The check on this compares the filename (from when it was imported), EXIF capture date & time and the file size - these have to all be the same to be picked up as a suspected duplicate.
 
Thanks for your reply. I will have to run some more checks on what was happening with my files. I assume it compares all those details to the original, unaltered file that was imported, even if I have made changes since Lightroom can always go back to the original. However, today I was running Import on some files from an old backup and I wanted to see if the files from that backup were in my LR catalog. Several times LR Import indicated some of them were not in LR but when I went back into LR it was there already. When I started looking at the subject on the internet there were quite a few messages about LR not always being correct in determining whether or not a file had already been imported. I suppose there may be some things that could cause one of those details to change enough that LR would not indicate it had already been imported.
