Why are all my folders on the left side of the library module by date and not folder names?

When you import, in the Destination panel on the right side of the import dialog, you choose "By Date". This option tells LR to create and mainatin a date oriented folder structure underneath with the folder selected just below. The pattern of the folder structure is selected on the following line (slashes mean that what follows the slash will be a new sub folder of what is before the slash. If desired, you can later rename these folders in the folders panel but that will not affect what LR does with any subsequent imports.

The other option is "Into One Folder". In this option you control the fulder structure and the names of the folders manually.
